Setting up an iframe using Form Builder

1. From left-side menu go to Forms > Choose your form

2. In the Properties tab of the form you will choose an iframe template

The html template may have specific styling  to match your organization's theme/website. If you'd like to create one please reach out to support.

3. Enter the host page URL and click Submit

The host page URL is the page which will host the iframe on your website. It is a page which has the same design and "wrapping" as the rest of your site but the form will be embedded in the page.
Your website administrator will be able to help you set up a host page.

4. Embed the form on your page

Click the icon next to "Put this code in your iframe host page". Copy the code and paste it into the iframe on your host page. Now this form will appear on your website.
