Ability to assign different default subscription options by form

We, and probably other users, have a good number of different forms we use to reach different types of stakeholders, interested parties, etc. It would make sense to be able to assign, on a form by form basis, different default subscription options.

For example, people who are signing up to receive our Green Clean-related news, that option (Green Clean E-Newsletter (quarterly)) should be checked by default... but for other forms we would not want that to be the default option.

  To clarify, these are forms which people use to sign up for (electronic) publications in our DataBank.
