Under the Setup menu, click Form Builder.
Select the "+" icon.
Select a form type from the dropdown.
Enter a unique form ID for internal organization. Your constituents won't see this.
Submit a title for your form. This will be seen by your constituents.
Duplicate
To copy an existing form:
Click the Edit icon of the desired form.
Select the Duplicate.
